With the passage of House Bill 2002 in Oregon, insurance carriers are also now allowed to use the ages of spouse and dependants as a factor when generating rates for group health insurance. In order to provide you with the most accurate quotes, we need to collect this information as well, including spouses and dependants who may be waiving coverage.

We do not disclose any non-public personal information about our customers or former customers to any non-affiliated entity except as described below and otherwise permitted by law. We may disclose all of the information we collect, as described above, to insurance companies or other companies that assist us in the servicing or administration of the product(s) or services you have requested or authorized.
